This tool reads a PDF's existing embedded text layer. A PDF made from scanned images with no OCR applied has no text layer to convert, so try the PDF OCR tool first on that kind of file.
How does it decide what's a heading?
It measures each line's actual rendered font size against the document's most common body text size — lines that are noticeably larger become bold headings automatically.
